Chaperone proposal and consent form.

This is required for all minors (under 18) when attending a competition as a member of the ESSU if their parents or lawful guardians are not in attendance.
Proposed Chaperones must sign this form to show that they have read the 'Chaperone Guidelines' and consent to act as such.
