Parental consent for nominated Chaperones.

All athletes under the age of 18 attending a competition when representing the ESSU, if not accompanied by their parent or lawful guardian, must nominate an appropriate Chaperone.  Parental consent to the arrangement must be obtained and verified by completing this document, a signed copy of which must be sent to the ESSU Secretary or the person nominated as Manager or Organiser of the ESSU team before travelling to the competition.
